We are available to see patients during our regular office hours:  Monday through Thursday  8:00 AM to 4:30 PM - closed from noon to 1:00 PM -  and Friday 8:00 AM until noon.  Please call our office (850) 771-2001, to schedule an appointment or fill out the contact form available on our website.  Along with office evaluation and consultations, we have on site Digital X-Ray services.  We make every effort to be considerate of your time and ask that you give us 24 hours notice if unable to make a scheduled appointment.  Our goal is to provide all our patients with the highest level of care in a professional environment and utilizing the latest technology, find the right solution for treatment of your joint, bone and muscle health in a convenient and efficient manner.   

Important Information

New patients please arrive at least a half hour early to ensure we have all the documentation necessary for your scheduled visit.  All patients should bring a photo ID/Drivers License, Insurance Card, up to date medical records, and a list of all current medications (prescription and over the counter) - we recommend you bring your actual prescriptions in their labeled container to ensure we have your current dosage.
